We would like to introduce our friends and family rewards program. For every person introduced by you to our practice you will receive a $50 gift card to use towards any dental treatment at Pershing Family Dental.
This program is our way of saying thank you for your trust & loyalty. We appreciate how you help our family practice continue to grow through your recommendation to your friends, family, and acquaintances.
Please feel free to fill out the form on the right hand side and refer anyone of your contacts. When they schedule their appointment and come into the office for any dental treatment, we will send out your gift card in a thank you note through the mail.
